New Patients

Our practice no longer accepts families who refuse all immunizations due to the risk that unimmunized individuals pose to others.  Our immunization schedule takes into consideration recommendations from the American Academy of Pediatrics and Centers for Disease Control Advisory Committee on Immunization Practices.   

We are thrilled that you have chosen to trust us with the care of your newborn! 

Please call our office at 865.687.1940 at your earliest convenience so we can verify that we are in-network with your insurance. Please download the new patient forms (found below) and fill them out ahead of time at home to bring to your first appointment. Once the hospital is ready to discharge the baby, call us and we will find an appointment time within the guidelines the hospital physician requests.

If you would like to become a patient at Drs. Black & Benton, please download and fill out the following forms. Once you have completed the forms, please send them with a copy of your insurance card (front and back) to us by:

• in person at our office between 8am and 5pm Monday - Friday.

Once we have processed your paperwork, we will call you to schedule your first appointment with us.
